To initiate automatic payment of your weekly or monthly donation gift to Sacred Heart please open the authorization form below and fill it out.  If you have any questions please call Rachelle at 319-283-3743.  After you have completed the form, please return it along with a deposit slip or voided check.  There are currently two easy ways for you to accomplish this: 

1) Scan the document and your canceled check or deposit slip and email them both to:  parish@sacredheartoelwein

2) Fill out the document and mail it along with the canceled check or deposit slip to the Sacred Heart Parish Office at:

 The parish now has a lock box at the entrance of the Parish Center.

You can drop off any envelopes or other there at any time.  It is checked on each business day.
